Tesamorelin activates GHRHR through Gαs-coupled adenylyl cyclase stimulation, leading to cAMP/PKA-mediated phosphorylation of CREB and downstream GH gene transcription and secretion in anterior pituitary somatotroph cell models. The trans-3-hexenoic acid N-terminal modification has been characterized in published research as a strategy for conferring DPP-IV protease resistance, making tesamorelin a longer-acting GHRHR agonist relative to native GHRH(1-44) in cell culture contexts and a useful probe for receptor occupancy duration studies.
